Medicaid Provider Listings
This is an online database provided by the state of Kentucky of healthcare providers - including dentists - who accept Medicaid/Passport.
Click here to search the database. You must make a selection on the “Are you looking for a Waiver Provider only?” question before the rest of the form will work. Under "Provider Type" dentists are listed under 60 - Dentist Individual and 61 - Dentist Group. After you make that selection, you can then use the Specialty dropdown if you need to search for a specialist.
